Under the leadership of Florida Accountable Care Services (FACS), several forward thinking Independent Physicians came together to form Central Florida Physicians Trust (CFPT) in order to Clinically Integrate themselves in preparation for the Future of Medicine. CFPT is moving towards further integration while remaining a network of Independent Physician Providers, as we feel that Independent Physicians remain the last bastion of truly patient-centric affordable healthcare. Beginning in January of 2013, CFPT is well on its way of achieving Medicare’s Three Part Aim: Better health for Individuals, Better health for the Population; and lowering the overall Costs of Care for our Patients.
Through our association with Florida Accountable Care Services and its numerous partner organizations we have been able to exchange best practices with several other ACOs and work towards our shared goals under a single unifying Voice, as well as developing additional platforms to bring our Physicians and Patients higher levels of individual quality care.
MISSION & GOALS
To implement physician directed and reality based patient-centric care coordination interventions that enhances the quality of care provided to Medicare Fee-for-Service beneficiaries while at the same time combating the pronounced escalation of Medicare Program costs.
The established goals of the CFPT Medicare ACO for care coordination are consistent with the contemporary movement toward reducing fragmentation and escalating costs within the health care system, and more specifically the published aims of the CMS Innovations Center and MSSP of “Better Health, Better Healthcare, and Reduced Costs”.
